The process of
Making a referral

Step 1: Complete the Online Referral Form

You will be taken to the secure Microsoft form by clicking the website button on the referral form to make a referral. Complete this form as fully as possible with information about the reasons for seeking the referral in the expanding boxes available.

Ask the specific questions you wish to have answered, or choose from the list of relevant questions provided. The assessment will cover up to 5 questions in the fee cost. Additional practitioner time can be allocated to answer more questions. This will incur a further charge.

Step 2: Triage and Appointment Offer

We review the form and send an appointment invitation to the employee, copying in the referrer. If the referral requires escalation to the OH Physician, we’ll discuss this with you first, as the fee will differ.

Step 3: Appointment Confirmation

After the employee confirms the appointment, you will receive a confirmation email with an attached invoice.

Step 4: Occupational Health Assessment

For the assessment call, the employee must have a private and confidential space to speak freely without being overheard.

The Occupational Health assessment includes a comprehensive review of the employee’s medical history to understand how it impacts the workplace role.

Step 5: Occupational Health Report

Following the assessment, a report is completed to offer information regarding the health concerns, advice on fitness for the work role and potential adjustments that the business may wish to consider.

The report will include information provided by the employee, shared medical letters, evidence-based medical information, employment law, and health and safety considerations.

We also discuss with the employee the importance of lifestyle factors to promote overall health, management of long-term health symptoms and well-being.

Our reports are released to incorporate the GMC guidance for Gold standard practice. The reports will be shared with the employee once the invoice has been settled. They have 48 hours to request changes to any factually incorrect information in the report. No changes to the Occupational Health opinion within the report will be accepted.

Once the employee has confirmed their agreement to share the report, it will be forwarded to the referring manager. If we do not receive a response from the employee, you will be informed so you can discuss the matter with the employee.

Convenient telephone or virtual assessments are a great option, offering flexible appointment scheduling and minimising time away from work. Occupational Health assessments typically involve gathering medical history and conducting verbal evaluations, taking around 30-45 minutes.
