Please complete the secure Referral Form below to request an Occupational Health Assessment. The form is brief and includes questions for you to select to have answered in the clinician’s report after the assessment. 

The assessment cost covers a maximum of 5 questions, which can be selected from the suggested options or specified by you. This flexibility allows you to focus on the areas that matter most. If more than 5 questions are needed, we can answer these and additional costs will apply.

Once the form is complete, it will be sent directly to our Admin team for processing. Within 2 working days of receipt of the referral form, the employee will be contacted with a suggested appointment. Please get in touch with our team if you have any questions or need further assistance.
