Once the Occupational Health Assessment Referral form is completed and submitted, it will be sent directly to our administrative team for processing. When making the referral, please consider the information you provide to ensure we fully understand the business concerns with the employee’s health and work. We will contact the employee within 2 working days during office hours with a proposed appointment time. 

When completing the referral form, you can choose from a list of suggested questions or present your own. Many clients prefer a combination of both, as this flexibility enables you to focus on the most relevant areas. We can accommodate more than 5 questions, which are included in the standard fee, though additional costs will apply.
